OSHA cites Ashley Masonry after 2012 death

FORT PAYNE, AL (WAFF) -

OSHA has cited a Fort Payne company for nearly $12,000 after three men fell from a school construction site, killing one person.

Ashley Masonry received five citations; four of them were considered serious.

One man died and two others were injured at the construction site of the new Crossville Elementary School after they fell two stories from some scaffolding they were working on in December, 2012.

According to the Occupational Safety and Health Administration, three of the citations dealt with the scaffolding, saying it was not constructed properly, lacked braces, and also lacked a complete guardrail system.

The fourth serious citation dealt with a forklift boom operator operating the machinery while not properly seated.

OSHA also gave the company a fifth citation not considered serious for having lifting or hauling equipment without its rated capacity posted.
